Job Duties:
The Registered Dietitian provides therapeutic nutrition services to participants of the WIC program with high risk and/or complex nutrition-related medical conditions. Nutrition services include assessment of nutrition and health, obtain anthropometric and hemoglobin measurements interpret results, provide one on one and group nutrition education and counseling, develop nutrition care plans for participant, refers participants to other health, and social programs as needed, promotes and supports breastfeeding, and documents in the MIS system, SCWIC.
Develops individual care plans for participants who are referred and deemed at high risk for poor health and/ or nutritional outcomes. Uses Nutrition Care Process for assessment, diagnosis, intervention, monitoring, and evaluation of participants. Schedules participants for follow-up, as needed.
Oversees food and formula prescriptions and coordinates with medical providers, as appropriate. Assesses and approves need for List II therapeutic formulas when ordered by medical provider. Builds food prescriptions in SCWIC for participants needing List II therapeutic formulas.
Provides nutrition education, including breastfeeding promotion and support that is responsive to the identified needs/interests of participant.
Promote and support breastfeeding to pregnant and breastfeeding women. Counsel breastfeeding women on the management of low or high risk nutrition related medical conditions. Issue and monitor breast pumps and breastfeeding supplies.
Documents providing referrals and conducting appropriate follow-up to referrals to high risk participants. Tracks high risk participants progress in improving their health and documents outcomes. Performs certifications, recertifications and nutrition education as scheduled.
Provide WIC certifications including health history, anthropometrics, immunization screening, hemoglobin or hematocrit testing, evaluation of risk status, prescription of WIC food package, documentation, and referrals following program standards and protocols.
Provide and document individual nutrition education counseling and follow-up for participants with low risk medical conditions.
Plan, conduct, and evaluate group nutrition education classes in accordance with WIC guidelines and agency policies.
Document all nutrition education contacts according to WIC policies and procedures.
Perform hemoglobin as needed in accordance with CLIA and agency standards.
Other duties as assigned to include but not limited to:
Update professional skills and knowledge to assure delivery of quality nutrition services. Complete and maintain advance certification(s) required for the position.
Participate in WIC trainings, region in-service programs, and agency required trainings to enhance related knowledge.
Consultation, Technical Assistance and Outreach
Prepares and submits reports and documents as required.
