The Special Supplemental Nutrition Program for Women, Infants and Children (WIC) is a public health nutrition program under the USDA providing nutrition education, nutritious foods, breastfeeding support, and healthcare referrals for income-eligible women who are pregnant or post-partum, infants, and children up to age 5.

You represent you are or will be fully vaccinated against COVID-19 as defined by the CDC prior to the start of 2022 Nutrition Education & Breastfeeding Promotion Conference and Exhibits (Conference). Proof of vaccination is required to enter the Conference. In consideration for being allowed to participate in the Conference, on your own behalf and on behalf of all the members of your household and immediate family, you release from liability and waive your right to sue or otherwise seek relief from the National WIC Association, its officers, directors, employees, volunteers and agents (collectively “NWA”) for any and all claims, including claims of NWA's negligence resulting in any physical injury, illness (including death) or economic loss you may suffer due to contracting COVID-19 and/or any related illness while attending the Conference and/or any events incidental to the Conference.
Our Conference provides attendee lists to our exhibitors and sponsors as a benefit of their support. I permit the National WIC Association to share on a one-time usage in a spreadsheet format my name, title, company, and mailing address. Email and phone numbers are never shared.
